Outreach Trainers

As we continue to develop and refine our digital infrastructure, we have updated our submission process to a fillable PDF format to enhance accessibility and consistency. Outreach Training Program Reports can now be completed within a single document for a more structured and coordinated submission process. Please download the correct industry's OTPR form using the document list below. Email completed reports to oshaed@uc.edu.

After submission, please allow up to 3 business days to receive your invoice via email. Credit card payments can be completed upon receipt of the invoice by calling 1-800-207-9399. If you are paying by check, please mail payment according to the instructions provided on the invoice. It may take up to 30 days for your student completion cards to be mailed.
