ADHD Record Requirements

What constitutes adequate medical documentation for establishing ADHD care at UHS?

  • Full report / results from neuropsychological testing clearly establishing a diagnosis of ADHD.
  • Records from licensed psychiatrist, pediatrician, or family doctor who has both established the diagnosis of ADHD through testing and has been prescribing and coordinating appropriate care for ADHD over time, preferably greater than 6 months.

What does NOT constitute adequate medical documentation?

  • “Quick” or “rapid” ADHD assessments
  • Online ADHD “quizzes” or “tests”
  • Rapid online ADHD care without documented, significant accompanying in-person evaluations.

How do I send my records to UHS?

Fax records with a cover sheet stating “ADHD Review” to University Health Services at 513-556-1337. The cover sheet or records should also include your UC id and UC email address.

What are the next steps?

Once records are reviewed, UHS staff will reach out to the patient to determine if ADHD care can be rendered either in the student health clinic, through UHS psychiatry, or if further testing / records are required. Patients with additional mental health diagnoses such as depression, anxiety, bipolar disorder, or schizophrenia may be referred directly to psychiatry.
